Guideline-Directed Medical Therapy (GDMT) is the gold standard for treating heart failure — a carefully tailored regimen of medications like ACE inhibitors, beta blockers, and diuretics proven to improve survival and reduce hospitalizations. But even the best treatment plan is only as effective as a patient’s ability to follow it.
Unfortunately, adherence after discharge is alarmingly low. Many patients struggle to keep up with their medications, monitor side effects, or recognize early signs of decompensation. For providers, this creates blind spots that lead to preventable readmissions.
CardioLink bridges that gap.
By combining biometric tracking with an optional pill dispenser and real-time alerts, CardioLink keeps providers connected to what’s happening between visits. When a patient misses a dose or their weight spikes overnight, the system flags it — enabling timely intervention before the situation escalates.
